Apartments run from a 330 square foot suite through a 400 square foot deluxe suite to one-bedroom homes between 378 and 552 square feet. Memory care offers a 330 square foot suite and a 400 square foot companion suite. Pricing is behind an inquiry form rather than published, so expect to ask. Respite care is worth noting separately for anyone reading this as a caregiver rather than for themselves. It covers a trial stay, recovery after a hospital admission, or simply a break, and a trial stay is a genuinely useful way to find out how a community treats a couple or an individual before anyone commits to moving. This community is listed here because a published guide to LGBTQIA+ senior housing names it as offering an inclusive, discrimination-free environment. That is a third party's characterization rather than a credential the community holds, and it is not the same as documented staff training. Ask directly what training staff receive and whether the nondiscrimination policy names sexual orientation and gender identity.

Compass on the Bay is a memory-support assisted-living community in South Boston, and one of the few communities in the Boston area given over entirely to people living with memory loss and dementia rather than running a memory unit alongside general assisted living. It holds Purple Flag accreditation for person-centered dementia care. Programming is research-based and covers social engagement, learning, fitness and a Brain Healthy Cooking program. The building is designed around cognition rather than around a hotel lobby: private apartments, small-scale community rooms, bright lighting and garden space. Staff receive LGBTQIA+-affirming training, which matters here more than almost anywhere, because a resident with dementia may not be able to advocate for themselves about who their partner is. Grace Ridge is a life-plan community, also called a continuing-care retirement community, on a 52-acre campus in Morganton, North Carolina, near the Catawba River greenway and the town center. All four levels of care are on one campus: independent living, assisted living, memory care and skilled nursing. That is the core of the life-plan proposition, and it is worth asking exactly how a move between levels is triggered and priced. Community life leans on arts and the outdoors, with painting, chorus, theater, volunteering, greenway access, dining and wellness programming, plus the Table Rock Lounge. Entrance fees and monthly charges are not published. Life-plan contracts vary widely in what they guarantee, so ask what is refundable, what happens if your money runs out, and what care costs once you need it.

Ivy Park at Lake Merritt is a memory-care community at 468 Perkins Street in Oakland, beside Lake Merritt and close to downtown. It is devoted exclusively to residents living with Alzheimer's, dementia or other cognitive changes, in private and companion studios, and it is licensed by California as a residential care facility for the elderly under RCFE number 019201599. Its programming is branded EverYou and is organized around five themes the operator describes as connect, create, fun, fit and learn, with activities from gardening and baking to live music and outings. This listing needs a piece of history attached to it, because the name has changed and the reason it is in this directory belongs to the old name. As Lakeside Park, under Watermark Retirement Communities, it was reported in 2019 as the first exclusively memory-care community in California to earn the SAGECare Platinum credential, meaning at least 80 percent of leadership and staff had completed SAGE's LGBTQIA+ aging training. The Human Rights Campaign also lists Lakeside Park, at this address, as a Long-Term Care Equality Index High Performer. Both of those belong to the previous operator. The community has since been rebranded as Ivy Park at Lake Merritt under Ivy Living, and Ivy Living's own page for it does not mention SAGE, SAGECare, HRC or LGBTQIA+ care anywhere. SAGECare is renewed annually and measures the share of a specific operator's staff who have trained, so it does not travel with a building when it changes hands. We have therefore not given this community a credential badge. If you are considering it, the useful question is a direct one: ask whether current staff hold SAGECare training under the new operator, and ask to see it. A yes would be worth a great deal here. Joshua Springs is an Avista Senior Living community in Bullhead City, Arizona, in the Colorado River region near the Nevada border. It offers assisted living, memory care and short-term respite stays, so a resident whose needs change does not necessarily have to move. Respite is worth noting separately: a short stay is often how a family tests whether a community fits before committing, and it is also the main practical relief valve for a caregiver who needs a break. Kendal at Oberlin is a nonprofit life-plan community in northeast Ohio, next to Oberlin College, open since 1993. Its residents come from 39 states, which is unusual and mostly explained by the college: music, art and lifelong learning are the reason many people move here. Care runs from independent living through assisted living, memory support, short-term rehabilitation and long-term nursing care, all on one 110-acre campus with walking trails and an arboretum. The community is resident-led in a real sense, with an active Residents Association, and the wellness and fitness center anchors day-to-day life. The Kendal organizations grew out of Quaker practice, which shows in the emphasis on self-governance. The office is open Monday to Friday, 8am to 5pm. There is a waitlist, managed through the community's portal, and a roadmap-to-residency program for people planning ahead. Pricing is not published.

Coler Specialty Hospital and Nursing Facility sits on Roosevelt Island in the East River and is part of NYC Health + Hospitals. It is large, with roughly 1,025 beds, and specializes in chronic care, physical rehabilitation, geriatrics, HIV and AIDS care, and Alzheimer's care. The HIV and AIDS specialism matters directly to this directory. A significant number of long-term survivors are now aging with the virus, often with complex comorbidities, and very few long-term care facilities have real depth in that combination. Coler is publicly listed as a 2025 Long-Term Care Equality Index Leader by the Human Rights Campaign Foundation, reflecting documented LGBTQIA+ policies and staff training. That is a policy designation, not a clinical quality rating. As a public facility, Medicaid, Medicare and system financial assistance apply. Confirm services and admission directly, and check the current inspection record on Medicare's Care Compare.

The Ivy at Golden Gate is an assisted living and memory care community in San Francisco, housed in a designated historical landmark building on 19th Avenue in the Sunset district. It offers assisted living alongside Evergreen at Ivy, the operator's specialist memory care program for residents living with dementia or Alzheimer's. Homes are studio, one-bedroom and two-bedroom apartments. The community is licensed by California as a Residential Care Facility for the Elderly, license number 385601148, which you can look up through the California Department of Social Services. Checking that record yourself is worth ten minutes for any assisted living community you are seriously considering. Pricing is not published. The Loft at Linn is an affordable assisted living community in East Providence, Rhode Island, specializing in memory care for people with a dementia diagnosis. It is run by AldersBridge Communities, a Rhode Island nonprofit, in the former Linn Health and Rehabilitation building. It has 22 private studio apartments laid out in a secure circle. That layout is a deliberate design choice rather than an architectural quirk: walking is one of the most common and least harmful expressions of restlessness in dementia, and a circular floor plan lets a resident walk continuously without meeting a locked door or a dead end. Amenities include spa-style bathrooms, an activities room, a beauty and nail salon, a barbershop, transportation, behavioral health support, outpatient therapy, secure elevators and life-enrichment programming. A memory garden was listed as coming in 2025. The word affordable is the community's own. Pricing is not published, so ask specifically how it is funded and whether Rhode Island Medicaid participation applies to your situation.
