Home care is the route most people actually take, and there is no national register of affirming agencies to send you to. What there is, is a way to find out: interview agencies the way you would interview a person, and ask the questions that reveal whether they have thought about you at all.

Find out what your state pays for

Every state runs home and community based services through Medicaid that can pay for care at home, and the rules and waiting lists differ enormously between them. Your Area Agency on Aging is the one phone call that finds yours.

Community that is not local

Get the paperwork right

This is the part that matters most where there is no affirming provider, and the part people leave latest. Whoever ends up making decisions for you is decided by documents, not by who loves you. Without them, a hospital falls back on next of kin, and chosen family are strangers to that rule.
